Historically, before the mid-1970s, when there was a need to establish and quantify ongoing care for chronic medical conditions, physicians and rehabilitation professionals were consulted to answer three fundamental questions: the patient's condition, the necessary care for that condition, and the cost of that care over time. Initially, there was significant variability in the quality and reliability of these evaluations, indicating a clear need for more structured and standardized life care planning.
Subsequently, organizations like the International Commission on Health Care Certification (ICHCC) and the American Association of Nurse Life Care Planners (AANLCP) have developed their own practice standards and professional credentials.

Standards of Practice

Standard 1: Qualification

- Formulate medical and rehabilitative opinions within their expertise, collaborating with other professionals as needed.
- Base medical opinions on diagnostic conditions on all relevant, objective findings from the discovery and analysis phase.
- Consider the full scope of potential physical and mental effects of diagnostic conditions when assessing impairments.
- Evaluate disabilities by examining the possible impact of impairments.
- Use peer-reviewed standards to determine care durations.

The NEW YORK PHYSICIAN SOCIETY Board Certified Physician Life Care Planner Certification Board follows rigorous policies and procedures to maintain fair, consistent governance and management across areas such as auditing, confidentiality, compliance, certification and recertification, and disciplinary measures.
Auditing
The Certification Board review all documentation proving eligibility for the NEW YORK PHYSICIAN SOCIETY Board Certified Physician Life Care Planner Exam. Candidates' eligibility depends on meeting the board's set standards. Candidates receive written notification of their eligibility status. If found ineligible, candidates can appeal in writing to the Certification Board within 30 days, providing evidence to support their case. The Certification Board exclusively reviews appeal information and makes a final eligibility decision.
For recertification, Professional Testing Corporation randomly selects individuals for audit twice a year, requiring them to demonstrate ongoing eligibility and continuing education achievements. The results are reported to the Certification Board.

Upon request, the Certification Board will confirm a holder's certification status to third parties, including the original award date, renewal date, and any expiration dates. This information is also publicly accessible on the Certification Board's website.
The board will also disclose any suspensions, revocations, or disciplinary actions against current or former certificate holders upon request but will not release any applicable individual exam scores.
Compliance
The Certification Board investigates all reported misconduct by certificate holders, candidates, or applicants. Written, notarized allegations must be sent via certified mail within 120 days of the purported violation, accompanied by supporting evidence. Misconduct allegations can range from certification ineligibility and applicable examination irregularities to professional negligence and legal convictions related to life care planning.
Disciplinary Action
Following an investigation into alleged misconduct, the Certification Board will:
- Conclude the investigation and impose any disciplinary actions or sanctions within 60 days.
- Any appeals against these decisions must be submitted in writing within 30 days.
- An appeal hearing panel will be formed and review additional evidence, concluding with a written decision.
All decisions and appeals are final and communicated to all involved parties via certified mail.
Suspension & Revocation
Certification may be suspended or revoked, or candidacy for the exam may be denied, suspended, or revoked due to application falsification, failure to maintain necessary medical licensure and certifications, failure to timely renew certification, or proven misconduct.
The Appeals Committee offers a route to contest suspension or revocation decisions, with all appeals needing to be written and notarized within 30 days of the decision. The committee responds to appeals within 30 days, and its decisions are final.

Petition for Certification through Affirmation of Physician Life Care Planner experience and holding an Active Board Certification from the American Board of Physical Medicine & Rehabilitation.
- Active medical licensure as an MD or DO.
- A minimum of 10 years of post-residency clinical practice in their medical specialty.
- At least 10 years of Board Certification in one of the ABMS-recognized medical specialties.
- Encouraged but not required is an Active CLCP™ Certification from the International Commission on Healthcare Certification (ICHCC).
- Authorship of at least 25 life care plans, verified by Physician Affirmation.
- A minimum of 20 instances of sworn expert testimony in life care planning, verified by Physician Affirmation.
- Absence of any professional sanctions for ethical or standard violations.
The Certification Program is managed by the Certification Board as described above.
RE-CERTIFICATION:
Recertification Process
Successful candidates of the examination and board review are entitled to use the designation after their names and will receive certificates from the board. A registry of certified professionals will be maintained and may be included in board publications.
The certification is valid for five years, after which the individua may be audited or required to submit eligibility and board review or fulfill alternative requirements available at that time to maintain their certification.
For recertification without re-examination, candidates must provide evidence of 60 continuing education units, 15 of which may be CME credits related to Physiatric coursework in physician life care planning.
Recertification applications, along with supporting documentation, must be submitted as follows:
- Applications and fees must be received one month before certification expiration.
- Continuing Education credits must be documented with a Completion Certificate detailing the course hours, provider, title, date, and location.
- Candidates must maintain an active, unrestricted medical license, an optional CLCP® Certification through ICHCC, and Board Certification by ABPMR over the preceding 5 years.
Late applications incur a $200 fee in addition to the standard recertification fee. Applications received over 30 days past expiration result in loss of designation, requiring retesting at full examination fees.
Candidates are responsible for keeping records of all continuing education documentation for five years.
Continuing Education for Recertification
The renewal credit system promotes ongoing professional growth, allowing for a range of educational activities. Sixty credits are required every five years for renewal, with documentation of completed activities essential.
Credits may be earned through:
- Approved life care planning or medical education, with one hour equating to one credit. Course outlines should be pre-approved by the Board.
- Presentations, with 5 credits awarded per presentation, up to 10 credits per renewal period. Presentation outlines require pre-approval.
- Research and publication in life care planning or clinically relevant areas, with 5 credits per item, up to 10 credits per period. Submissions must be pre-approved.
- Item writing for test questions, with 1 credit for every 5 accepted questions, up to 10 credits per period. Submissions should be evidence-based and are subject to board approval.
- Board or optional committee participation in AAPLCP™ or the Certification Board, with 5 credits per year of verified participation, up to 20 credits per period.
APPEALS:
Appeals Process
The NEW YORK PHYSICIAN SOCIETY Board Certified Physician Life Care Planner Certification Board welcomes appeals related to audit findings, compliance matters, disciplinary measures, and cases of suspension or certification revocation.
For appeals concerning eligibility audit outcomes, the appellant must provide a written appeal to the Certification Board within 30 days of receiving the eligibility denial. Supporting documentation for the appeal should be included with the submission. The Certification Board exclusively reviews appeal materials and makes a final decision on eligibility. All decisions made by the Certification Board on appeals are conclusive.
In cases involving disciplinary actions, sanctions, suspensions, or revocations, the aggrieved party must submit their appeal in writing to the Certification Board within 30 days following receipt of such notice. Upon receiving an appeal, the Certification Board will examine any supplementary evidence and establish an appeals hearing panel within 30 days. This panel will meet within 60 days of formation, concluding with a written determination. All parties involved will receive the panel's verdict via certified mail. The outcomes of all appeals, as decided by the Certification Board and the appeals panel, are definitive and binding.
