Membership Application - NEW YORK PHYSICIAN SOCIETY

BEFORE WE BEGIN

By submitting this application, I express my desire to join the NEW YORK PHYSICIAN SOCIETY. Should I be accepted, I commit to adhering to the society's regulations.

Please ensure that every part of this application is filled out completely before submission. Incomplete applications, due to missing information, will not be reviewed.

We assure you that all provided information will be handled with the utmost confidentiality.

Inclusion of a recent, clear headshot is mandatory.

While not obligatory, a recommendation letter from an existing society member is highly recommended.

The NEW YORK PHYSICIAN SOCIETY's current membership contributions and initial fees are structured as follows:

  • Standard Membership: Annual dues of $3,850 with a one-time initiation fee of $1,000.

  • Medical Staff Membership: Annual dues of $2,750 with a one-time initiation fee of $750. Applicants must be below 35 years of age at the time of acceptance.

  • Corporate Membership (Non-Physician): Annual dues of $5,000 with a one-time initiation fee of $5,000.

All dues and initiation fees are subject to relevant taxes and service charges.

Allocating roughly ten minutes should suffice to complete this form.
